Attention Polar Bear Nation: Free sports physicals and vaccination clinic is happening at Fairmont Senior on Wednesday, May 6. The Health Dept RV should be here at approximately 9:00 am. Attached are the NEW (updated 2026) sports physical form and the MCHD vaccine consent form. ONLY students who have forms COMPLETED & SIGNED BY PARENTS will be able to receive services at these clinics. We MUST have forms IN-HAND on the day of the clinic. Completed forms are REQUIRED and physicals are invalid if not using the 2026 form. The forms are available in the main office. Go Bears!
