The goal of MATCH is to change health in West Virginia for the better, but we can't do it without your help. Complete the MATCH survey and ensure that your voice is heard!

- Improve health for West Virginians

- Make sure health resources are in WV communities most in need
- Help policy makers and health officials better understand the reality of health in your community
HOW DO I PARTICIPATE?
One in 14 West Virginia adults will be randomly selected to take the survey. If selected, you will receive an invitation with a choice on how you would like to complete the survey:
- Online survey
- Mail-in survey
- Call-in survey
